Person-centered integrated care delivery for patients with chronic kidney disease and multimorbidity in the primary care setting and primary-nephrology care interface

What is the study about?
We would like to better understand the level of integrated kidney care and care coordination in Alberta from the perspectives of adults with kidney disease, caregivers, and health care providers. Person-centered integrated care puts people, not diseases, at the center of health care systems. The goal is to empower patients to take charge of their health by providing collaborative, coordinated, accessible, and individualized care.
Why is this study being conducted?
Patients in Canada with chronic kidney disease and those that care for them have identified person-centered integrated care as a research priority area, but there is little research on this type of care for patients with mild to moderate chronic kidney disease and other health conditions. By measuring the level of kidney care coordination in Alberta, we hope to understand what is working and highlight the gaps that can be addressed in future phases of our research.
How will the study be completed?
We are sharing an online 15-minute survey based on a validated set of questions (based on the Rainbow Model of Integrated Care). This survey will ask participants located in Alberta to describe their experience with kidney care. Participants can also complete the survey on the telephone by calling the coordinator at 403-210-8820.
Participants are invited to complete a 15-minute survey with questions about their experience with kidney care in Alberta. At the end of the survey, there will be demographic questions. All questions are optional and participants can end the survey at any time.
