Person-centered Approaches to Transforming Healthcare in CKD

Overview
People living with CKD in Canada have said that person-centered, integrated care is a top research priority. However, there is limited research about how to deliver this kind of care—especially for people with early-stage CKD and other chronic health conditions. This study will help clarify where improvements are most needed and support the development of a more coordinated, patient-centered approach to kidney care.
Navigating the healthcare system can be especially difficult for people living with chronic kidney disease (CKD) and other health conditions like diabetes, hypertension, or cardiovascular disease. Many people with early-stage CKD receive care from both primary care providers and kidney specialists, but the system is often fragmented.
This study aims to identify the most important challenges to person-centered, integrated kidney care in Alberta. We will use a research approach called a modified Delphi to gather insights directly from those with lived or living experience of CKD—including patients, caregivers, and healthcare providers.
What will participation involve?
Participants will be asked to complete four short activities—all online, completely remote:

A 5-minute online demographic survey

A 60-minute online meeting to discuss challenges in kidney care

Choose between Thursday, May 15 at 9:00 AM or 4:00 PM MST

A 10-minute online survey in May to select the top 10 challenges

A 10-minute online survey in June to rank those challenges
Participants who complete all activities will receive a $50 gift card as a thank-you for their time.
Eligibility
To participate, you must:

Live in Alberta

Be 18 years of age or older
We are looking for:

Adults with early-stage CKD and other chronic conditions (e.g., diabetes, hypertension)

Caregivers of adults with early-stage CKD and other health conditions
Please note: People who have received dialysis or a kidney transplant are not eligible for this study.
